A Licensed Practical Nurse (LPN) Coordinator handles the day‑to‑day coordination of the team clinical functions, assists in implementing the patient plan of care with staff, volunteers, and outside agencies, communicates as a liaison between staff, patients, and outside agencies, assists with maintaining records, and may involve direct patient care.
Essential Duties and Responsibilities
Assist in coordination and implementation of the patient plan of care.
Handle phone calls from the patient, primary caregiver, or designee regarding any questions or concerns.
Collaborate with the interdisciplinary teams, RNs, or Team Manager.
Coordinate and schedule patient visits.
Provide timely documentation/data entry per Hospice time frames and deadlines.
Communicate current health status and needs of patients to appropriate team professionals.
Assist the team and Team Manager with other administrative functions, as necessary.
Qualifications
Graduate of an accredited School of Practical Nursing with current LPN licensure in the State of New York.
1 year of nursing practice as a Licensed Practical Nurse preferred.
Salary: $27.00 per hour
