Essential Duties and Responsibilities

Provide skilled nursing care to patients as directed by the IDG plan of care in accordance with agency policies, including personal care, vital signs, and medication administration.

Document patient care to agency standards with timely completion and signatures, following the A-I-R protocol.

Reinforce patient and family teaching regarding disease process, self-care techniques, and prevention strategies under the guidance of the assigned registered nurse.

Assist in planning and exchanging information with other health professionals to maintain continuity of patient care.

Assist the physician and registered nurse in performing specialized procedures.

Identify and report patient or family complaints or unusual occurrences, including incident reporting.

Demonstrate proper sharps disposal, hand hygiene, waste disposal, and use of personal protective equipment.

Communicate current health status and patient needs to appropriate health professionals, contributing to the coordination of the overall care plan and discharge planning.

Participate in quality improvement programs and document required quality assurance data.

Participate in weekend, holiday, and backup visits as needed.

Adhere to agency policies and procedures focused on patient care and organizational administration.

Maintain compliance with HIPAA regulations and ensure security of patient electronic and written records.

Qualifications

Graduate from an accredited School of Practical Nursing with current LPN licensure in the state of New York.

Valid driver's license with no serious violations; vehicle required for on-duty or on-call use.

Preferred: 1 year of nursing practice as a Licensed Practical Nurse.

Salary: $27.00 per hour.
