Position Summary:
This role is ideal for a
Physician Assistant (PA-C)
seeking work-life balance, autonomy, and a deeper patient connection. You'll round at various senior communities and play a vital role in delivering
coordinated, relationship-based care .
Responsibilities:
Visit patients in assisted living and memory care settings
Provide physical exams, manage ongoing conditions, and adjust care plans
Collaborate with physicians and support staff to optimize treatment
Educate families and facility teams on care coordination
Qualifications:
MN-licensed PA in good standing
Experience in senior care, home visits, or family medicine
Excellent documentation and interpersonal skills
Valid driver’s license and vehicle required
