This opportunity is for County Caseworkers to build rapport and a professional relationship with consumers of all ages in services and their families. Serve as an advocate for the consumer, monitor the delivery of services, link individuals to services, develop social supports, and encourage self‑determination.

DESCRIPTION OF WORK This professional position is responsible for providing case management to individuals meeting criteria for developmental delays, mental health, and/or intellectual disabilities. Responsibilities include:
Monitoring, linking, and evaluating services.
Referring to other services as needed.
Providing regular direct individual contact and face‑to‑face case management services.
Meeting productivity standards set for the program.
Scheduling and facilitating meetings to develop an individualized service plan with involvement of the individual and/or family.
Entering necessary information and maintaining information in computerized database, state computer systems, and charts in accordance with state, federal, and licensing regulations and Agency policy.
Obtaining and updating information within required timeframes.
Providing crisis intervention services as needed.
Cooperating with and consulting with Agency staff, supervisors, other providers, and community resources as needed.
Linking individuals to transportation services or transporting them to appointments as needed.
Attending meetings, staffing, and trainings as needed.
Providing coverage in Somerset County as required.
Performing other related duties as assigned.
WORK SCHEDULE AND ADDITIONAL INFORMATION
Full‑time employment.
Work hours are 8:00 AM – 4:00 PM, Monday – Friday, with a 30‑minute lunch.
On occasion, staff may need to work outside of the set work week to complete projects, meet work demands or in the event of an emergency.
Hours may vary to meet the needs of the consumers.
This position reports to the DBHS Office in Bedford for work. At times may provide coverage in Somerset County.
Telework: On probation (6 months) and training, staff are eligible for hybrid of on‑site and remote work after 3 months.

REQUIRED EXPERIENCE, TRAINING & ELEGIBILITY QUALIFICATIONS
Two years of experience as a County Social Services Aide 3 and two years of college level coursework, including 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences.
A bachelor’s degree with or supplemented by successful completion of 12 college credits in the same fields.
Equivalent combination of experience and training which includes 12 college credits in specified disciplines and one year as a County Social Services Aide 3 or similar paraprofessional case management functions.
Applicants will be considered to have met the educational requirements within two months of graduating with a qualifying degree or credits.
OTHER REQUIREMENTS
PA residency requirement is currently waived for this title.
Must be able to perform essential job functions.
LEGAL REQUIREMENTS
Must pass a background investigation.
Position falls under provisions of Child Protective Services Law and requires satisfactory criminal history reports.
Position falls under provisions of Older Adult Protective Services Act and requires satisfactory criminal history reports.
