The Case Manager - CMCT will provide responsive, solution-focused, internal/external customer service that supports case management functions. The Case Manager - CMCT will ensure inquiries are routed to the appropriate departments; follow-up as needed. The Case Manager - CMCT will support with quality assurance tracking and data and assist with completing and maintaining case management documentation. The Case Manager - CMCT will provide case management services and support to temporarily assigned individuals; support with department and team coverage as needed. TheCMCT is also responsible for checking the Afterhours shared inbox during regular business hours and ensuring that VMs are responded to and/or routed to appropriate CM or other staff in a timely manner

This role offers a 4-day, 40-hour workweek, Monday-Thursday, and includes the flexibility to work remotely, as well as the opportunity to collaborate in-person at our headquarters office in Aurora, CO, and/or at off-site locations.

Requirements

Bachelor's Degree; or Five (5) years of experience in the field of Long-Term Services and Supports; or (LTSS), which includes Developmental Disabilities; or Some combination of education and relevant experience appropriate to the requirements of the position

Valid Colorado motor vehicle license and provide proof of motor vehicle insurance Meet our driving requirements - no suspensions and no more than two (2) tickets in the past three (3) years

Upon request, qualifying need, and approval, driving requirements may be waived in lieu of reliable transportation.

Preferred

One (1) year of experience in developmental disabilities/long-term services and supports Able to speak, write, and interpret fluently in English and another language based on business needs Ability to complete all typical case management responsibilities in fluent language/s Support with written translation of internal documents for use by stakeholders, as appropriate

Additional Responsibilities

Complete required documentation related to monitoring and monitoring activities Ensure effectiveness of services delivered (as authorized by the service plan) and ensure identified outcomes are met, advocates for individual service needs Facilitate interdisciplinary teams (IDT) meetings to coordinate services to meet individual's needs; ensure appropriate interaction and communication; implement IDT meetings as appropriate to modify services Complete assessments to determine eligibility for services/supports and identify individual strengths, needs, concerns, and preferences through interviewing, observing, and utilization of standardized tools Schedule, facilitate, document, and complete follow-up for all required meetings; disseminate updates and obstacles to the necessary team members Make collateral contacts with such community resources such as law enforcement and medical personnel to obtain additional information, coordinate services, or secure their assistance Serve as subject matter expert for applicable programs and services; serves as resource/point of contact for complex cases Maintain knowledge of state/ federal regulations, policies and procedures regarding long-term services and supports, Medicaid waiver and state-funded programs and other community resources Maintain a professional and ethical manner in all interactions and meets performance quality, customer service, and coordination standards as assigned by department leadership; maintain appropriate boundaries
