has an opportunity for a

Program Manager (PM) - Active Case Management in our

Case Management Department

to supervise a team of individual contributors and engage their staff, peers, and leadership with integrity and authenticity and are responsible for creating and maintaining an engaged and supportive team culture. The Program Manager - Active Case Management will support and cultivate a team that primarily functions as case managers, assuring the delivery of quality case management services to individuals applying for, and/or receiving services, and their families. The PM will provide technical support and direction to ensure case management services are delivered, due process is followed, and confidentiality is maintained. Also, providing the needed level of support to all supervised staff in various circumstances/settings including emergency and crisis situations.

This is a 40-hour, 4-day work week (Tuesday-Friday) hybrid position working primarily remotely, but coming into our headquarters office in Aurora, CO, and/or other offsite locations as needed.

Requirements

A Bachelor's Degree; Or five (5) years of experience in the field of Long-Term Services and Supports(LTSS), which includes Developmental Disabilities; Or some combination of education and relevant experience appropriate to the requirements of the position

A minimum of two years of experience with mentorship, team lead, and/or direct supervision Working knowledge of the Medicaid Long Term Care system Experience managing sensitive information with discretion Valid Colorado motor vehicle license and provide proof of motor vehicle insurance Meet our driving requirements - no suspensions and no more than two (2) tickets in the past three (3) years

Upon request, qualifying need, and approval, driving requirements may be waived in lieu of reliable transportation.

Preferred Project management experience

Additional Responsibilities Supervise and direct activities of assigned staff to ensure timely processing, submission, and distribution of reports, data, and work. Fosterand maintainan inclusive team with productive team cohesion and dynamics Observe, directly supervise, evaluate, and document performance and assure performance evaluations are completed and reviewed for all assigned staff. Address staff disciplinary issues in consultation with the Associate Director and Human Resources Coach and mentor staff to improve the quality of documentation, deadlines, and soft skills, as needed Participate in the hiring process for open positions, as needed Lead and collaborate with others on all aspects of project management, with emphasis on stakeholder communication, resource availability, and impact to the community Ensure all applicable policies, procedures, and regulatory guidelines regarding persons served, processes, and funding are met. Ensures timely and efficient completion of documentation for funding continuity Ensure workloads and/or caseloads are covered in the event of absenteeism and/or staff vacancies /monitor workloads, as appropriate Successfully complete and update required training; participates effectively and appropriately and ensures training completion for staff Respond to complaints and resolve issues to the best outcome possible

We are a designated Case Management Agency (CMA) for long-term care services and are one of Colorado's Community Centered Boards (CCB), connecting people to federal, state, county, and private funding.
