Work Days & Hours: Contingent Position
Any day Monday - Friday, set own hours
Paid travel time, paid mileage and pay per visit rates
3-4 admissions/week
On-Call Rotation: Yes
1 summer holiday, 1 winter holiday and 1 weekend a month
What you will do as a Physical Therapist with Shared Services Home Health Care
Review request for physical therapy and physicians orders.
Interview client to determine type of treatment and schedule therapy.
Evaluate type of therapy most desirable after consulting with physician.
Assist clients in ambulatory training as necessary.
Recommend modifications and changes in the clients therapy treatment as indicated.
Ensure that all therapist notes are informative and descriptive of the care provided and of the clients response to the care.
Select rehabilitative and therapeutic activities and exercises based on medical and social history of clients.

What it takes to join our team
Doctoral degree in Physical Therapy from an accredited college or program
Must possess an active license to practice as a Physical Therapist in the State of Michigan in good standing
1 year of homecare experience preferred
Experience with Wellsky/Kinnser and Forcura preferred
