Job Summary Baptist Health is seeking a Registered Nurse (RN) for its Hospital at Home program, allowing patients to complete their hospital stay at home. Patients receive remote patient monitoring (RPM), technology kits, and communication devices. A 24‑hour command center staffed by nurses monitors patients virtually, and contracted personnel provide in‑home care when required. This program is a transformational initiative in healthcare delivery.
Shift Details PRN, 6 shifts over a 6‑week period, released 6 weeks in advance, with rotating weekends and holidays.
Location Hospital At Home Contact Center located in Downtown, Jacksonville.
As a Registered Nurse, You Will Be Responsible For
Provide virtual patient care for individuals located in Downtown and across the Baptist Health system.
Deliver comprehensive patient care through all phases of the nursing process — assessment, planning, implementation, and evaluation.
Organize and supervise care provided by supportive personnel while coordinating care with ancillary services.
Educate patients, significant others, and families on disease processes, treatments, and care plans.
Serve as a patient advocate to ensure high‑quality, compassionate care.
Administer treatments and medications as prescribed and monitor patient responses.
Manage care for patients with a variety of medical, surgical, and chronic conditions, as well as minor injuries or illnesses.
Promote Baptist Health’s mission of delivering excellence in care through empathy, safety, and teamwork.
Education Required Associate’s Degree – Nursing.
Education Preferred Bachelor’s Degree.
Experience
2 years Acute Care Nursing Experience Required.
License and Certification
Basic Life Support (BLS) Required.
Advanced Cardiac Life Support (ACLS) Preferred.
NIH Stroke Scale Certification Preferred.
Certified Medical‑Surgical Registered Nurse (CMSRN) Preferred.
Certified Acute‑Care Virtual Registered Nurse (CAVRN) Preferred.
Licensed Registered Nurse Required.
