After Hours Nurse – LPN or RN (8p – 8:30a)

Join our team as an On‑Call Nurse to positively impact the experiences of patients and families during evening hours. You will have the autonomy to utilize your nursing skills when patients and families need help after hours in urgent times. Your schedule is 8p – 8:30a, with an expectation to work every other weekend and every other holiday, covering a multi‑county territory. This position includes a $3,000 sign‑on bonus for full‑time work.
What You Will Do
Assess physical, psychosocial and spiritual needs of the patient and family (RN only).
Gather data related to patient and family’s needs (LPN).
Provide intermittent skilled nursing services under the patient’s physician’s direction.
Monitor and evaluate pain and symptoms; provide information and assistance to manage problems.
Serve as a liaison to the patient’s physicians when needed and provide relevant information.
Provide emotional support to the patient and family.
Refer patients and/or family members to other services when appropriate.
Provide instruction (health care teaching) related to care in a residence.
Make a visit to the patient’s residence at the time of death.
Maintain, promptly complete, and return all documentation as required.
Communicate and act as liaison with patients, family, and health professionals respecting confidentiality and dignity.
Relay pertinent information to appropriate team members.
Participate in meetings as initiated by the team leader.
Participate in mandatory or elective HWR University events, trainings, or in‑services as necessary.
RNs shall participate in triage coverage as needed.
Perform other duties as assigned.
Success Capabilities
High School Diploma or equivalent required.
Graduate of an accredited school of nursing; Bachelor’s degree preferred.
Licensed RN/LPN in the state of Ohio.
Experience preferred in pain and symptom management, oncology, or mental health.
Ability to communicate effectively, orally and in writing.
Ability to work independently and collaboratively with a team.
Must have demonstrated knowledge and skills to provide appropriate care to all age groups.
Ability to handle stressful and constantly changing conditions; must make sound professional decisions quickly.
Ability to access patients and families in any residential and/or public setting in any weather condition.
Proficiency in Microsoft Office (Word, Excel) and related technology.
Must have and maintain a valid driver’s license, good driving record, and acceptable insurance.
Physical Requirements
Ability to stand and walk for extended periods.
Manual and finger dexterity and eye‑hand coordination for nursing care and equipment operation.
Ability to lift up to 75 lbs occasionally, and 50 lbs frequently using safe transfer techniques.
Ability to stoop, squat, or bend frequently.
Corrected vision and hearing within normal range.
Possibility of exposure to environmental elements, bloodborne pathogens, chemicals, allergens, and inclement weather.
