Licensed Practical Nurse (LPN) Nights, FT or PT
Hospice of The Western Reserve is dedicated to providing compassionate, high‑quality end‑of‑life care. We seek an experienced LPN to serve a multi‑county service area with 24 or 36 hours per week. Key Responsibilities
Practice in accordance with HWR clinical standards, policies and procedures. Assess physical, psychosocial and spiritual needs of patients and families. Deliver intermittent skilled nursing services under physician direction. Monitor pain and symptom management, providing guidance to patients and families. Act as liaison to physicians, delivering pertinent information on patient needs. Provide emotional support to patients and families. Refer patients/families to additional services as appropriate and follow‑up. Teach health education to patients and families. Maintain all hospice documentation accurately and in a timely manner. Participate in care conferences, bereavement follow‑ups, case reviews and quality improvement activities. Supervise activities of registered nurses and assist with onboarding. Participate in on‑call rotation and discharge planning with the interdisciplinary team. Required Qualifications
Active OhioPN license. Preferred experience in hospice care, home health, oncology, or pain/symptom management. Valid CPR certification. Valid driver's license with acceptable driving record and auto‑insurance coverage. Hospice and Palliative Care certification preferred; required within the first two years of hire.
