Base Pay $19.00/hr – $21.00/hr
Position Overview The Lead Community Mental Health Worker oversees and coordinates services for individuals receiving support in the community. The role guides clients to achieve independence, supervises and mentors Community Mental Health Workers (CMHWs), and ensures compliance with client goal plans, statutory requirements, and program policies. The Lead also manages the 24‑Hour Emergency Assistance program and works collaboratively with internal and external partners to enhance client outcomes.
Role Responsibilities
24‑Hour Emergency Assistance (EA):
Responsible for the 24‑hour emergency program Monday‑Friday during business hours; respond promptly to emergency calls, provide on‑call counseling and crisis intervention; perform emergency visits; maintain detailed documentation; serve as the key point of contact for staff.
Supervision & Team Leadership:
Mentor CMHWs; assist program manager with supervision and team development; facilitate regular team meetings; provide direct guidance during crises; coach CMHWs in crisis management techniques.
Client Care & Support:
Provide support for implementation of IHS plans; ensure Goal Plans, CSSP, and IAPP are aligned; monitor progress toward goals; facilitate regular client meetings and documentation; teach independent living skills; aid clients in coping strategies and crisis counseling.
Program Oversight & Compliance:
Ensure compliance with program policies, statutory regulations, and the 245D statute; manage a caseload of IHS clients; communicate regularly with clients, families, and stakeholders; maintain accurate records in RTI's EMR system.
Administrative & Documentation:
Review and approve weekly progress reports and incident documentation; ensure all client documentation is up to date; perform administrative tasks such as charting and office coordination; guarantee documentation adheres to internal and external guidelines.
Collaboration & Communication:
Serve as primary contact for clients, families, and external providers; foster collaboration with internal staff and external partners; facilitate ongoing communication between program staff and key personnel; address concerns regarding abuse, neglect, or exploitation.
Professional Development & Staff Training:
Provide ongoing training and professional development opportunities for CMHWs; participate in staff meetings and training sessions; assist with onboarding of new team members; maintain current knowledge of best practices.
Other Duties:
Perform additional duties as assigned, including additional on‑call rotation participation.
Required Qualifications and Experience
Associate degree in Behavioral Sciences, Social Work, or a related field from an accredited institution.
Experience with mental health care and community services in independent living and goal planning.
Certification in First Aid and CPR (or willing to obtain).
Must possess a bachelor’s degree in social work, psychology, or a closely related field; or a minimum of three (3) years of relevant experience. Equivalent education/experience may be considered.
Must pass the required background check and remain eligible to provide services as defined by the Minnesota Department of Human Services.
Must pass the screening required by the Federal Human Services Office of Inspector General.
Must have a reliable vehicle, valid driver’s license, and insurance.
Preferred Qualifications
Bachelor’s degree in one of the behavioral sciences or related fields.
2000 hours of experience delivering clinical services for mental illness or children with emotional disturbances.

Use hands for writing, typing, and data entry.
