Schedule Options: 7.5, 10, or 12 hour shifts | Full-time, Part-time, Per Diem.
Position Summary
The LPN is responsible for delivering the highest quality of care and services to residents by safely administering medications and treatments, supervising CNAs, and supporting daily living activities. This role ensures compliance with Crouse Community Center’s mission and policies while maintaining a safe, organized, and compassionate environment.
Key Responsibilities
Medication Administration & Documentation:
Administer medications and treatments using the Six Rights of Medication Pass: right resident, medication, dosage, time, route, and right to refuse.
Ensure medications are passed on time and documented accurately.
Record refusals or missed doses per protocol.
Maintain strict confidentiality of resident information and medication records.
Count and verify narcotics with another Medication Care Nurse.
Resident Care & Communication
Monitor residents’ conditions and assess needs to provide optimal care.
Review and update the Daily Log to stay informed of resident status.
Report resident concerns to the Nurse Manager during medication rounds.
Demonstrate and promote the mission and values of Crouse Community Center in all interactions.
Supervision & Compliance
Oversee and support Certified Nursing Assistants (CNAs).
Admit, transfer, and discharge residents as needed.
Practice standard precautions for hygiene, cleanliness, and infection control.
Ensure all care and services align with facility policies and regulatory standards.
Requirements
Qualifications:
Must possess a current license to practice as an LPN.
Long term care experience.
Must be eligible to work in LTC per the NYS HCS guidelines.
Ability to handle multiple priorities.
Previous experience working with seniors preferred and desire to serve and care for seniors.
Ability to make choices, decisions and act in the resident’s best interest.
Ability to react and remain calm in difficult situations.
Possess written and verbal skills for effective communication and level of understanding.
Demonstrates good judgment, problem solving and decision-making skills.
