An overview of this position entails…This position is responsible for providing intensive case management services to children and families and operates as part of a broad case manager team with the overall goal of increasing self‑sufficiency for families. The position will be responsible for coordination and facilitation of services and the related documentation. Some assigned roles may be responsible for Supervised Family Time (SFT) duties which will involve observations and facilitation of parenting time with interventions as appropriate and/or directed, ensuring safe parent/child interactions. Case management services may include initial assessment, treatment planning, direct service delivery and education, information and referral, recommendations to external entities, follow‑up, and discharge planning, as required for the position.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Meet and provide direct services to clients: build relationships, provide necessary and appropriate advocacy, communicate clearly and directly, provide information and education. Coordinate, identify and facilitate needed services or referrals to achieve overall goals of reducing or preventing confirmed child abuse or neglect, preventing out of home placement or current placement disruption, encouraging positive participation in education or job skills training, and increasing overall family, client and youth self‑sufficiency.

Complete all necessary documentation for enrolled clients, including admission paperwork, service plans, service plan reviews, and monthly summaries. Maintain ongoing records of all case management contacts. Create progress summaries and share as necessary with the participating agencies.

Complete discharge paperwork which is prepared for the discharging party at the time of discharge from the program. Complete final discharge summaries within the defined period of time, and appropriately share the summary with the participating parties. Create after‑care recommendations, referrals and contacts.

Ensure all necessary billing information is provided for accurate billing in a timely fashion.

Ensure each client’s file is accurate and contains all pertinent information.

Facilitate and/or attend all necessary staffing for each client/client family. (These staffings may occur in multiple locations with multiple agencies covering multiple topics. Having a general awareness of the services continuum and how it impacts clients is important.)

Maintain all necessary paperwork and releases that enable clients to safely and legally participate in the program.

Represent Hilltop as a service provider in court proceedings, as required.

Prepare timely billing summaries and spreadsheets in accordance with Hilltop procedures.

EDUCATION & EXPERIENCE Two years of college or four years of related work experience; or equivalent combination of education and experience.

Certificate from Crisis Prevention Institute (CPI) (preferred)

Competitive Pay and Benefits

The starting wage range for this role is $22.00 to $23.10 per hour.

Part‑time and full‑time schedules that can fit your needs.

The schedule for this specific position is part‑time schedule of Sunday 9:00 am–5:00 am, Monday–Thursday 3:00 pm–4:00 pm. Some daytime hours may be available depending on participant needs.
