Autumn Lake Healthcare at Union is seeking a full‑time Physical Therapist to join a skilled nursing facility and provide outstanding, individualized care.
Overview The Physical Therapist evaluates patients to identify needs and establishes an individualized treatment plan of care. By motivating patients during treatment, you will assist them in functioning optimally while promoting a healthy lifestyle through improvement of strength, flexibility, balance, and coordination. This role is ideal for therapists who enjoy being part of a cohesive group, are seeking thorough, continuous development, and would like the ability to involve in team decisions.
Responsibilities • Evaluate patients and establish individualized treatment plans.

Qualifications
An active state Physical Therapist license is required.
Unlimited employee referral bonus of up to $1,000 each.
Option for PRN coverage at multiple locations with potential full‑time opportunities at additional sites.
Direct hire position; no agency involvement.
Open to PT candidates requiring H‑1B sponsorship, including current student visa holders, H‑1B transfers, and new H‑1B filings.
