Job Summary:

The Case Manager provides direct care and support services to the adult crisis population. The Case Manager will assist in the development and implementation of discharge planning activities for individuals in the Crisis Stabilization Unit. The Case Manager will help the CSU and the treatment team link individuals to aftercare services, engage them in aftercare, and conduct follow‑up contacts to improve active participation in after‑care services. The Case Manager will collect intake information, conduct admissions based on interviews with individuals, their families, significant others, and appropriate community agencies, help the treatment team identify strengths and weaknesses, and develop after‑care plans for specific activities, services, programs, or supports.

Job Duties and Responsibilities:

Assist individuals with identifying aftercare needs and developing a discharge plan through interviews with the individual and family when appropriate, the treatment team and clinical staff.

Participate in treatment team meetings to assist with discharge planning needs.

Provide linkage to aftercare services by arranging appointments, transportation and supports.

Provide follow‑up and assistance to improve engagement in aftercare services and reduce recidivism.

Monitor the individual's physical and emotional well‑being and report unusual behavior or physical complaints to direct supervisor or Charge Nurse.

Assist in the admission and discharge process, including obtaining vital signs, a safety search of the individual's and the individual's personal belongings, and orienting new individuals to the unit.

Maintain clinical records and documentation according to agency policies.

Assist in prevention and management of crisis situations; respond appropriately to crisis situations that may involve physical intervention including the use of seclusion/restraint.

Provide direct care for individuals as needed.

Transport individuals to approved designated location at discharge.

Other duties as assigned.
