Job Title Nurse: RN or LPN (Crisis Stabilization)

Overview Highland Rivers Behavioral Health provides treatment and recovery services to individuals impacted by mental illness, substance use disorders, and intellectual developmental disabilities. The Registered Nurse (RN) and Licensed Practical Nurse (LPN) are responsible for the delivery of quality individual care through the nursing process of risk assessment, admissions, discharges, administering medication, unit management, and crisis management on the Crisis Stabilization Unit (CSU), a 24‑hour, short‑term medically monitored unit with 28 beds.

Job Duties and Responsibilities

Complete all admission and discharge paperwork, including nursing assessments and care plan development for individuals in crisis.

Monitor all aspects of individual care, report concerns to the APRN and CSU Nurse Manager.

Record and report symptoms and changes in conditions by observation, documenting progress or lack thereof.

Complete required daily shift notes for all individuals on CSU.

Ensure all chart checks, CIWA, COWS, vitals, and required logs (refrigerator, temperature logs) are completed on the shift.

Transcribe orders if needed; complete daily assignment sheet; review and update BHL board for accurate census.

Maintain stock of supplies in the department.

Provide education to individuals and families in line with Release of Information policy.

Facilitate groups and activities on the unit.

Assist with service‑related compliance and intervene in acute crisis situations.

Consult with other outpatient site staff regarding individuals.

Assist in training co‑staff as needed.

Maintain confidentiality per agency and HIPAA policies.

Demonstrate competency for blood glucose monitoring, alcohol breathalyzer, urine drug screen, pregnancy test, and PPD skin tests; monitor and document all ordered labs.

Perform safe phlebotomy practices when drawing blood labs.

Assist with seclusions and restraints; complete all required paperwork.

Attend case staffing/treatment team meetings; maintain clinical records per agency policies.

Ensure timely completion of agency‑required trainings/workshops and participate in meetings as needed.

Other responsibilities and tasks as may be assigned.
