Job Summary:
The Manager of Care Coordination reports to the Director of Care Coordination and leads a department of 28 full‑time equivalents (RNs and Clinical Care Associates). Overseeing daily operations, the Manager ensures efficient, compliant execution of the organization’s care coordination goals and achieves key performance metrics. The role also involves staff performance management, data analysis to improve performance, and participation in departmental projects to enhance patient experience.
Responsibilities
Organize and execute daily activities, staffing, and performance improvement initiatives for clinical services such as LOS reduction.
Implement care coordination activities in line with medical center policies, affiliated health plans, and regulatory requirements.
Oversee the department to ensure timely and compliant care coordination, discharge planning, utilization review, and social work interventions.
Collaborate with managers, physicians, administrators, and community health workers on care management issues identified through corporate or facility initiatives.
Manage staff performance, review data to drive improvement, and facilitate department meetings and projects.
Requirements
Minimum of 3 years experience in Care Coordination or Case Management.
Bachelor’s degree in Nursing or equivalent professional experience.
Current California RN license.
Compensation $60.24 – $89.60 per hour (based on experience and qualifications).

Location Mercy San Juan Medical Center, 384‑bed non‑profit Level 2 Trauma Center in Carmichael, CA. The facility serves north Sacramento County and south Placer County and offers comprehensive stroke centers, spine centers, and excellence certifications in multiple specialties.
