The position is a full‑time, health care provider role with a pay range of $60.24 to $89.60 per hour.
Job Summary and Responsibilities
Manages daily operations to ensure efficient and compliant execution of the organization’s Care Coordination goals and key performance metrics.
Oversees department staffing, performance improvement initiatives, and regulatory reporting.
Implements Care Coordination activities in accordance with the medical center’s policies and standard practices of the professions under the manager’s accountability.
Consults and collaborates with physicians, administrators, and community‑based health workers regarding care management issues identified through corporate or facility initiatives and current literature.
Advocates for the department by communicating organizational goals to staff and by bringing staff issues, requests, and new ideas to leadership.
Qualifications
Minimum of three (3) years of experience in Care Coordination or Case Management.
Bachelor’s degree in Nursing or equivalent experience required.
Current California RN licensure required.
Location Mercy San Juan Medical Center, 384‑bed Level 2 Trauma Center, Carmichael, California. The facility serves north Sacramento County and south Placer County, and is one of the area’s largest medical centers.
