Base pay range $43,500.00/yr - $50,000.00/yr
Location/Schedule:
Hybrid role with remote work and regular on-site requirements at the Crossnore, NC campus. Must live within one hour driving distance of Crossnore, NC campus. Typical work hours 8:30 am to 5:00 pm with flexibility for after hours and weekend scheduling. Participation in on-call rotation for referrals line.
Your Impact Crossnore Communities for Children is seeking a Referrals and Admissions Coordinator to join our centralized referrals and admissions team. This role is responsible for timely intake, screening, and coordination of admissions. You will serve as a critical first point of connection, helping children and families access the right care quickly and compassionately by guiding them through the referrals and admissions process.

Education & Experience
Qualified Professional qualification strongly preferred, which are:
Master's degree in a human services or related field from an accredited college or university and a minimum of one year supervised clinical experience working with children and families; or
Bachelor's degree in a human services field and two years' experience with the current population; or
Bachelor's degree in a non-human services field and four years' experience with the current population
Minimum of two years of professionally supervised experience working with children and families.
An equivalent combination of education and experience may be considered.
Skills & Competencies
Knowledge of child welfare and mental health systems
Experience with referrals, intake, or admissions processes
Strong customer service and communication skills
Ability to triage needs and exercise sound judgment
Ability to work effectively in crisis situations
Attention to detail and strong organizational skills
Ability to manage multiple tasks and meet deadlines
Proficiency with Microsoft Office and Google Workspace
Experience with Electronic Health Records (EHR) preferred
Ability to work with diverse populations and multidisciplinary teams
Key Responsibilities
Receive, screen, and triage referrals by phone and email
Collect and review required clinical, legal, and funding documentation
Communicate placement decisions and next steps to families and referral sources
Coordinate admissions and intake with clinical, medical, and program teams
Enter and maintain accurate records in the electronic health record and tracking systems
Verify insurance eligibility and funding sources (Medicaid, LME/MCOs, etc.)
Participate in after-hours and weekend on-call rotation for referrals
Collaborate with internal staff and external partners to support continuity of care
