About the Role We are seeking a dedicated and compassionate
Adults Social Worker
to join our
Integrated Discharge Team
at Southampton General Hospital. This role is key to ensuring
safe, timely discharges
and promoting
independence
for adults with complex needs.
What You'll Do
Hospital-Based Support:
Provide face-to-face assessments on hospital wards.
Community Work:
Carry out community visits to review hospital discharges (transport required).
Integrated Working:
Work closely with health professionals, families, and community services to ensure smooth, effective discharge planning.
Promote Independence:
Support adults in regaining control and confidence as they transition from hospital to home or ongoing care.
Assessment & Planning:
Complete strengths-based assessments, safeguarding involvement, and personalised care plans.
Multi-Agency Collaboration:
Contribute to MDT meetings, discharge planning discussions, and joint risk assessments.
