SUMMARY Under the supervision of the Program Manager and/or Team Leader, the Clinical Services Coordinator provides case management, rehabilitation, and crisis interventions and outreach services in home, office, and community settings to adults with serious long‑term mental illness. Collaborates with the psychiatric and allied mental health team in assessment, diagnosis, intervention and treatment planning. Conducts substance abuse treatment with clients in need in accordance with the 12 core functions of substance abuse counseling as defined by the ICRC. RESPONSIBILITIES

Provides clinical interventions such as resource linking, resource modification, development, skill teaching, and programming for consumers on the Mobile Treatment Team caseload.

Assists consumers in getting financial benefits.

Assists consumers in accessing medical and dental care as needed.

Assists consumers in choosing, getting, and keeping employment.

Assists consumers in choosing, getting, and keeping decent, affordable housing.

Assists consumers in developing a social support network and participating in social and leisure activities.

Assists consumers in applying ADL skills in needed real‑life environments.

Conducts comprehensive clinical assessment and develops interventions for substance abuse, sexual abuse, symptom management, and family support and education.

Assesses suicidal and homicidal behavior.

Coordinates emergency psychiatric services in a timely manner, including documenting interventions, alternatives considered, and responses of client, client family, and supporting agencies.

Provides, coordinates, and monitors treatment services for all assigned cases.

Meets a minimum of 22 hours of direct, face‑to‑face services with clients to adhere to productivity requirements.

Documents in progress notes following established procedures, as described in the Individual Treatment Plan.

Co‑ordinates care prescribed in the Treatment Plan through documented contact and implementation of assigned activities with other client, family, Gateway staff, consultants, and Treatment Plan‑designated staff from schools, human service, and medical organizations.

Participates in treatment planning by completing assessment assignments within time requirements specified by procedures.

Schedules and attends treatment planning meetings with staff and other individuals specified in procedures.

Utilizes assessment information, diagnostic results, and staff participation in the development of treatment plans.

Provides psycho‑educational services to clients and their families/significant others based on modality, frequency, participant and location specifications prescribed in the Individual Treatment Plan.

Provides information regarding mental illness, alcohol and substance abuse, effective parenting, and behavior management.

Provides therapeutic/counseling/case‑management services on time, alert, and responsive to client issues and behaviors.

Works within prescriptive instructions from the client Individual Treatment Plan, modifying treatment planning to meet client needs and changes in situation.

Documents, with the client when possible, treatment implementation and related issues in progress notes and/or treatment plan review documents.

Assists with discharge planning.

Ensures medical record documentation adheres to Gateway policies and procedures and MTT Medicaid guidelines, including updating the client database, reviewing outside correspondence, and following client confidentiality and rights procedures.

Attends scheduled community and/or agency meetings on time.

Makes arrangements with other agencies to provide services for identified clients as specified in client Treatment Plans.

Participates in agency committees and inter‑agency meetings as designated by the supervisor, attending on time and actively participating in discussion and assignments.

Participates in daily staff organizational meetings and treatment planning review meetings.

Provides ongoing assessment of client’s mental illness symptoms and response to treatment.

Provides ongoing assessment, problem‑solving, side‑by‑side services, skill training, supervision, and environmental adaptations to assist clients with activities of daily living.

Assists clients in finding and maintaining a safe, affordable place to live, including apartment hunting, roommate selection, landlord negotiations, cleaning, decorating, and procuring necessities (utilities, furniture, linens).

Teaches money‑management skills, such as budgeting and bill‑paying, and assists clients in accessing financial services, including professional financial counseling and emergency loan sources.

Helps clients access reliable transportation, including obtaining a driver’s license and vehicle, arranging for cabs, accessing bus lines, and finding rides.

Assists and supports clients in obtaining and effectively using a primary care doctor, dentist, etc.

Plans, structures, and prompts social and leisure activities on evenings, weekends, and holidays.

Prepares for supervision by completing assigned tasks and formulating questions.

Monitors medication to ensure client understands prescribed dosage, monitors side‑effects, and ensures compliance.

Attends weekly general staff meetings.

Performs other duties and responsibilities as assigned or needed.

MINIMUM QUALIFICATIONS

Associate’s degree required. Bachelor’s degree preferred.

Two years of experience in human services preferred.

Experience working with individuals with substance abuse, sexual abuse, and/or involvement with the criminal justice system is preferred.

NECESSARY SPECIAL REQUIREMENTS

Valid driver’s license and use of a car during normal working hours, or other means of transportation to complete major functions of the job.

Automobile liability insurance coverage for the vehicle being used in accordance with state regulations.

PAY RANGE $19.58 – $32.31
