Possess, as a minimum, an accredited Nursing Degree or a graduate of an approved RN training program and possess a current, unencumbered, active license to practice as an LPN in the State of Ohio. Prefer experience in a hospital, acute skilled nursing care facility, or other related medical facility. Be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long‑term care. Be willing to work harmoniously with other personnel and families and communicate effectively with residents at all behavior or acuity levels. Responsibilities

Promote residents’ well‑being and satisfaction by supporting resident‑centered care, developing and interpreting standards of care, and guiding the interdisciplinary team as it establishes and implements residents’ plans of care. Model and ensure that others provide medical treatment in a manner that honors resident preferences and individuality. Administer medications and perform nursing procedures according to established policies and nursing standards of care, ensuring all orders and changes are followed. Administer professional services such as catheterization, intravenous therapy, tube feedings, suction, dressing changes, etc. Monitor activities of daily living, assess mental, physical, emotional and spiritual well‑being, and encourage resident participation in activities. Document assessments and progress notes in the EMR accurately and timely, perform other administrative duties such as completing medical forms, evaluations, studies, and reports. Help plan, develop, implement, and evaluate activity/recreational programs for residents. Uphold the philosophy of Person‑Centered Care and provide purpose, choice and freedom to residents.
