General Description: The Healthy Coach will provide quality patient care to Premier Community HealthCare Group’s (PCHG) patients with the administration of treatments and medications, care for the ill or injured, and promote wellness, maintenance of health, and the prevention of illness of others under direct supervision of a registered nurse or licensed physician.

This position is grant funded by BayCare and requires case management of 100 diabetic patients in accordance with the clinical health coach training module, host and facilitate Diabetes Education Empowerment Program (DEEP) group educational sessions. Training will be provided by BayCare in the study protocol, Clinical Health Coach Training and as DEEP facilitators.

Essential Duties and Responsibilities:

Facilitate patient visits and ensures patients are taken care of in a timely manner and maintain high patient satisfaction. Includes monitoring and responding to task boxes.

Obtain quality measure data as a preparation for health care encounter and record in Electronic Medical Records (EMR)

Screens Pre-Diabetic and Type 2 Diabetic patients and enrolls a minimum of 100 patients in the Bay Care Grant

Completes monthly Healthy Living Coach patient education sessions per Bay Care Grant documentation requirements

Host and facilitate Diabetes Education Empowerment Program per requirements

Provide patient education for Type 2 Diabetic and Prediabetic patients enrolled into Healthy Living Program, along with providing education to non-enrolled patients as requested

Performs procedures and treatments following grant requirements and completes all orders and documentation per policy

Issue patient referrals per grant protocols

Record medical information and relevant notes in the patient’s chart timely and accurately reflecting all items obtained within visit. Ensure the correct data is captured and entered per grant requirements.

Ensure appropriate hand offs and flow trackers are completed to make patient visit run smoothly.

Effectively communicates with patients/families during complex or highly sensitive matters in a professional manner.

Perform CLIA waived and lab testing and transmission per Laboratory Protocols.

Perform venipuncture as required for blood collection per Laboratory Policies.

Collect and process body function samples as ordered and send out for lab testing.

Address routine patient complaints and builds strong rapport with patient population

Exhibits and promotes Core Values and is actively engaged in promoting a positive work environment and service excellence.

Ensures safety and compliance protocols are followed in accordance with PCHG and regulatory policies.

Proficient knowledge of PCHG policies and procedure and demonstrates appropriately and consistently. Understands resources and can effectively work towards solutions.

Ensure confidentiality and HIPAA compliance at all times

Keep other care team members informed when situations occur that disrupts timely patient flow through site.

Adhere to patient care needs and the core values of Premier Community HealthCare to a provide best-in-class patient experience

Work towards attaining department goals and drive Quality Improvement / Quality Assurance (QI/QA)

Qualifications:

Current Florida licensure for Licensed Practical Nurse

Current Basic Life Support (BLS) certification

Basic understanding of medical terminology and documentation
