(Required proficiency)What you will do* Provide case management services to pregnant, postpartum and inter-conception women, infants and families. Provide education and support to women before, during and after their pregnancy. Provide social determinates of health screenings, referrals for services and appropriate follow up. Provide perinatal mental health screenings, education, and referrals for services and appropriate follow up. Build relationships with patients, healthcare providers, agencies and other resources to best connect women and their families to helpful services.
