Lakeshore talent is in search of an Occupational Therapist (OT) for a direct-hire opportunity in San Bernardino, California.

Monday–Friday, 8:00 AM–4:30 PM (30-minute lunch) Pay Rate:

$44.66–$58.08 per hour Travel:

10%–25% travel to participant homes and other facilities as needed Mileage reimbursed; drive time included in hours worked

Required Qualifications Graduate of an Occupational Therapy program accredited by the American Medical Association and the American Occupational Therapy Association (AOTA) Bachelor’s, Master’s (MOT), or Doctorate (OTD) in Occupational Therapy National certification by AOTA or eligibility for the National Registration Examination Active state registration/licensure in the state of practice Valid state-issued driver’s license, reliable personal transportation, good driving record, and required auto insurance Minimum of one (1) year of experience working with frail or elderly populations Preferred Qualifications Two (2) or more years of experience At least one (1) year of occupational therapy experience in a geriatric setting

Job Summary The Occupational Therapist provides comprehensive occupational therapy evaluations and treatment plans for participants, delivering care across acute, restorative, and maintenance levels. This role collaborates closely with an interdisciplinary care team to support participant independence, safety, and quality of life.

Key Responsibilities Conduct occupational therapy evaluations and develop individualized treatment plans Integrate therapy plans into the interdisciplinary Plan of Care (POC) Provide skilled occupational therapy services as ordered by a physician Educate participants, families, caregivers, and staff on ADLs, safety strategies, exercise programs, and use of durable medical equipment (DME) Assist with ordering, inventory, distribution, and maintenance of DME Conduct home visits and educational sessions as indicated in the POC Document evaluations, treatment sessions, progress notes, and discharge summaries in a timely manner Report participant progress to physicians and appropriate care team members Participate in care planning meetings, staff meetings, and patient care conferences Perform data collection, reporting, and maintain participant statistics Support quality assurance, safety, environmental, and infection control programs Participate in peer review, utilization review, and performance improvement activities Maintain therapy equipment and ensure a clean, organized therapy department Report incidents, variances, or complaints immediately Perform other duties as assigned
