Monday–Friday: 8:00 AM – 3:00 PM

Position Overview An organization is seeking experienced

Community Health Workers

to support outreach, enrollment, and ongoing member services for the

CalAIM program . This role blends

community outreach and enrollment

with

case management–style responsibilities

and is best suited for individuals who are self-directed, comfortable working in the field, and experienced with CalAIM or similar programs.

Key Responsibilities Conduct community outreach to educate individuals on CalAIM services and enroll eligible members Coordinate and staff outreach tables at clinics, hospitals, and community-based events Build relationships with community partners and facilities to support outreach efforts Manage a caseload of up to 50 members (average caseload 25–30) Conduct member assessments and manage incoming referrals Provide case management support, including housing navigation as needed Travel within the community to meet members and conduct outreach Meet monthly enrollment expectations (minimum 20 enrollments per month)

Required Qualifications Prior outreach and/or enrollment experience with the

CalAIM program

or similar community-based programs Ability to work independently in the field with minimal supervision Reliable transportation and willingness to travel locally Bachelor’s degree (preferably in Social Work, Psychology, Sociology, or related field) with 2+ years of relevant experience

OR

no bachelor’s degree with 4–5 years of direct community health, outreach, or enrollment experience

Preferred Qualifications Community Health Worker (CHW) Certificate Bilingual English/Spanish Existing community outreach network or experience coordinating outreach locations
