This position requires the LVN to assess patient conditions, administer prescribed treatments, and monitor progress while collaborating closely with physicians, therapists, and other healthcare professionals. The role demands a strong commitment to patient education, helping individuals and their families understand care plans and manage chronic conditions effectively. The LVN will also be responsible for documenting all care provided accurately and timely, ensuring compliance with healthcare regulations and organizational standards. Ultimately, this position aims to enhance patient quality of life by providing personalized, high-quality nursing care in a home health environment.As a Home Health LVN, you will:Under the direction of the RN, assist in identifying the patient's physical, psycho social, and environmental needs as evidenced by documentation, clinical record, team report, and evaluations.Participate in planning and implementing care in conjunction with the RN, in accordance with the POC.Applies concepts of infection control and standard precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease.Assure the continuity of care through delivery of quality patient care.Provide effective communication to patient/family, team members, and other health care professionals as evidenced by clinical notes, case conferences, communication notes, and evaluations.Monitor assigned cases to ensure compliance with requirements of third party payor.Promote Agency philosophy and administrative policies to ensure quality of care.RequirementsCurrent LVN LicenseValid Driver's LicenseValid Auto InsuranceCPR CertificationHomeCare HomeBase experience preferredBenefits and PerksMedical, Dental, Vision Insurance401k with matchingHealth Savings AccountLife InsuranceShort & Long Term Disability InsurancePaid Time OffPaid HolidaysMileage Reimbursement/ Company cars could be available after 90 days of FT employment if all conditions and terms are met as assigned by the fleet care managment team.
