JOB SUMMARY: The

Regional MDS Coordinator (RN)

provides oversight, guidance, and support to multiple skilled nursing facilities to ensure accurate, timely, and compliant completion of the Minimum Data Set (MDS) process. This role partners closely with facility leadership, MDS Coordinators, and interdisciplinary teams to support regulatory compliance, optimize reimbursement, and promote high-quality resident care.

RESPONSIBILITIES:

Provide regional oversight of the MDS process across assigned skilled nursing facilities.

Ensure MDS assessments are completed accurately and timely in accordance with CMS, Medicare, Medicaid, and state regulations.

Monitor compliance with RAI guidelines, PPS schedules, and care planning requirements.

Support facilities in survey readiness related to MDS and documentation practices.

Review and monitor PDPM accuracy and supporting documentation.

Identify opportunities to optimize reimbursement while maintaining regulatory compliance.

Analyze quality measures, Five-Star metrics, and clinical indicators impacted by MDS.

Assist facilities with corrective action plans related to MDS, audits, or survey findings.

Provide training, mentoring, and ongoing education to facility MDS Coordinators and interdisciplinary teams.

Support onboarding and competency development of new MDS staff.

Serve as a clinical resource for MDS-related questions and best practices.

Partner with DNSs, Administrators, Therapy teams, and Business Office staff.

Participate in IDT meetings as needed to support accurate assessments and care planning.

Communicate trends, risks, and recommendations to regional and corporate leadership.

Conduct routine MDS audits and documentation reviews.

Track and report compliance, trends, and performance metrics.

Assist with internal and external audits as required.

Performs other duties as assigned.

REQUIREMENTS:

Current RN license in the state of practice.

Experience in a skilled nursing facility or long-term care setting.

Previous MDS experience preferred.

Excellent assessment, documentation, and care planning skills.

Proficiency in using electronic health records (EHR) and MDS software.

Excellent communication and interpersonal skills.

Ability to travel between facilities as needed.
