Key Responsibilities & Duties
In this role, you will be responsible for the overall coordination and management of services for assigned participants across multiple programs. This includes reviewing intake documentation, gathering additional information as needed, and developing individualized service plans (ISPs) in collaboration with individuals, legally authorized representatives, caregivers, support coordinators, and other stakeholders. The role involves completing annual assessments, maintaining accurate and comprehensive documentation, monitoring progress toward ISP goals through documentation review and on-site observation, and completing required quarterly reports. The Case Manager serves as the primary point of contact and advocate for participants and their support networks, attends and facilitates planning and problem-solving meetings, interfaces with community partners and agencies, and maintains current knowledge of community resources. Requirements & Qualifications
Qualified candidates will have a bachelor's degree in a related health or human services field, which is preferred; however, a combination of education, training, and experience may be considered instead of a degree, with a minimum requirement of a high school diploma or equivalent. Candidates must have relevant specialized training, certifications, and work experience, including at least two years of experience working with individuals with disabilities or medical concerns, with demonstrated experience in service management and related documentation. Strong knowledge of community resources, human services systems, and applicable regulations is essential, along with excellent communication, analytical, and documentation skills. The ability to work independently, manage multiple priorities, and travel between program locations using personal or alternative transportation is required. Hours & Schedule
