Responsibilities
Supports the organization’s mission by striving for excellence and fostering positive interpersonal relationships with co‑workers.
Assesses and evaluates the needs of patients with multiple disease processes and comorbidities and develops an appropriate plan of care.
Documents all changes in patient’s condition and updates care plans and outcomes.
Identifies teaching needs and plans appropriate strategies for individuals and families.
Monitors quality and appropriateness of services provided.
Establishes and maintains professional working relationships with health care providers and other members of the interdisciplinary team to provide patient and family care.
Adheres to agency infection control and safety policies.
Adheres to the organization’s policy regarding absenteeism and appearance.

Qualifications
Active New Jersey RN license.
Bachelor’s degree in Nursing is preferred.
1+ year of supervised clinical experience as a registered nurse.
Excellent verbal and written communication skills.
Strong computer skills.
Excellent organizational and interpersonal skills.
Car and valid driver’s license.
Working Conditions & Physical Demands Indoor and outdoor patient care with clinical equipment; use of phone and computer. Job characteristics include client and customer contact.
Pay Transparency In compliance with New Jersey’s Pay Transparency Act, the total compensation range for this position is USD $60,000 to USD $110,000. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate’s work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions.
