Job Summary
The Registered Dietitian is responsible for developing and implementing the WelbeHealth nutrition services program, working in close collaboration with the Interdisciplinary Team (IDT). The role includes delivering nutrition services such as assessments and counseling to participants, functioning as a liaison between WelbeHealth and food and supplement service providers, developing menus, and ensuring compliance with all regulations related to kitchen and meal service delivery. The dietitian collaborates effectively with colleagues and stakeholders to promote WelbeHealth values, team culture and mission.
Duties and Responsibilities
Conduct nutrition assessments to determine participants’ needs, preferences, and goals and actively participate in IDT meetings to develop care plans.
Deliver and document direct care nutrition interventions per physician orders or agreed-upon care plans, including implementation and monitoring of special diets, meal intake, and providing counseling to participants and families.
Oversee menu planning and meal delivery to ensure safety and regulatory compliance, and that participants receive correct special diets.
Coordinate supplements as ordered.
Provide staff training on the meal delivery program as needed.
Work with the PACE Physician and other IDT members to support smooth care transitions between settings (hospitals, skilled nursing facilities, home, etc.).
Support the intake and enrollment process to ensure participants progress through enrollment in a timely and effective manner.
Remain alert to input from team members, participants, and caregivers; update the IDT promptly about any changes in participants’ condition or psychosocial status.
Be legally authorized to perform in the position location.
Act within the scope of authority.
Be willing and able to work a varied schedule, including evenings, nights, and weekends.
Complete all required documentation in a timely and accurate manner.
Protect privacy and maintain confidentiality of company procedures and information about team members, participants, and families.
Follow WelbeHealth policies and procedures and participate in required quality improvement activities, staff training, and meetings.
Communicate regularly with supervisor and team regarding workload and priorities.
Complete all mandated trainings and education promptly.
Complete all mandated occupational health screenings as needed.
Exercise flexibility in performing assignments as business needs evolve.
Perform other duties as assigned.
Required Skills & Experience
Minimum of four years’ experience managing nutrition services in a similar setting.
Demonstrated ability to mentor and develop team members.
Minimum of one year of experience with a frail or elderly population.
Excellent organizational and communication skills.
Ability to work independently with minimal supervision.
Demonstrated ability to prioritize in a fast‑paced environment.
Experience and competency working with diverse backgrounds and cultures.
Commitment to unlocking the full potential of our most vulnerable seniors.
