Position Type:

Wage Range:
$45 – $55 per hour, depending on experience

Physical Therapist Responsibilities
Assess & diagnose: evaluate patients' physical conditions, movement abilities, and medical history to develop personalized treatment plans
Develop & implement treatment plans: create and execute rehabilitation programs to improve functional deficits of the patient
Provide hands‑on therapy: perform manual therapy, therapeutic exercises, and use modalities such as ultrasound and electrical stimulation as needed
Educate & support patients: provide patient, caregiver, and staff education as clinically necessary, including pain‑management techniques and fall‑risk safety; teach patients and families about exercises, injury prevention, and pain‑management techniques
Monitor progress & adjust plans: track patient outcomes, document progress, and modify treatment plans as needed
Collaborate with healthcare teams: work with doctors, nurses, and other therapists to ensure comprehensive patient care
Physical Therapist Qualifications
Graduate of an accredited Physical Therapy program and hold a bachelor’s degree
Current accredited MN Physical Therapist License
Strong knowledge of state, federal, and local regulations pertaining to home health care
Capable of performing the essential functions of the job, with or without reasonable accommodations
Fluent in English, both verbally and in writing, with sufficient communication skills to perform the required tasks
