We’re hiring a
Case Manager
in North Hollywood, CA. What You’ll Do
Build strong relationships with mental health providers, hospitals, and the Department of Mental Health Lead discharge planning and care coordination Apply Harm Reduction, Housing First, and Trauma-Informed Care principles Collaborate with medical teams to triage participants to appropriate care Conduct risk assessments and coordinate with PMRT when needed Provide crisis intervention, medication support, and conflict resolution Supervise MSW interns and support onboarding of new hires What We’re Looking For
Master’s degree required
in MSW, MFT or related field A minimum of 5 years of experience in case management, crisis intervention, and group facilitation Strong understanding of mental health systems and community resources Ability to work collaboratively in an interim housing setting (no field work) This is your chance to be part of a compassionate team that’s changing lives every day.
