What You'll Do: The Impact You'll Make As a
Clinical Case Manager , you will provide evidence-based, trauma-informed support to clients residing in Alcott's Interim Housing program. You'll guide individuals toward recovery and stability through clinical assessments, counseling, case management, and linkage to essential community resources.
Assessment & Treatment
Conduct comprehensive intake assessments and develop individualized treatment plans.
Facilitate individual counseling and group education sessions.
Evaluate client progress and update treatment goals as needed.
Therapeutic Services
Deliver interventions rooted in harm reduction and trauma-informed care.
Provide 1:1 counseling to support motivation, recovery, and stability.
Foster coping skills that reduce isolation, prevent relapse, and promote independence.
Care Coordination & Linkage
Connect clients with medical, behavioral-health, and substance-use services.
Coordinate community referrals and recovery-support activities.
Provide crisis intervention and respond to urgent needs in housing sites or the community.
Collaboration & Documentation
Participate in interdisciplinary team meetings and case conferences.
Maintain timely, accurate documentation in electronic health records (EHR).
Ensure compliance with all program, funder, and agency requirements.
Other Duties
Perform additional responsibilities as assigned to support program operations.
What You'll Bring: Your Qualifications Education & Experience
Master's degree in Social Work, Marriage and Family Therapy, Counseling, or related mental-health field (required).
Current
BBS registration (ASW, AMFT, or APCC)
within 90 days of hire (required).
Experience providing clinical services in substance-use treatment, corrections, or community mental-health settings (preferred).
Skills & Knowledge
Strong clinical, crisis-intervention, and documentation skills.
Familiarity with harm reduction, trauma-informed care, and motivational interviewing.
Excellent communication, organization, and teamwork abilities.
Proficiency in electronic health-record (EHR) systems.
Licenses & Requirements
Valid California Driver's License, clean driving record, and insurability under agency policy.
Successful
Live Scan background clearance.
COVID-19 vaccination and at least one (1) booster required
(medical or religious exemptions must be documented upon hire).
Bilingual (English/Spanish) strongly preferred.
