As an employee of the Fairfield County Board of Developmental Disabilities, the job incumbent shall comply with all Board policies at all times, and shall demonstrate respect for, support dignity of, and observe the rights of all individuals served by the agency. Duties
55% Service and Support Coordination
Acts as the primary point of coordination for the person-centered planning team. Utilizes an assess/plan/review/revise process to help teams plan for each person assigned. Assists individuals and teams to identify issues, areas of potential risk, and outcomes that are important to and for the individual. Completes and/or coordinates thorough assessments to determine the need for services and supports. Revises assessments as needs change and annually. Facilitates the development of a person-centered plan, including the determination and direction of needed supports. Revises plan as needed based on individual or team concerns, progress toward meeting outcomes, plan review, MUI prevention plans, or other issues that impact the need for services as described in the plan. Identifies funding sources for supports identified in the Individualized Service Plan (ISP) by utilizing a hierarchy of funding options applicable to each person. Plans with teams to access services that are cost efficient and effective to meet the needs described in the ISP. Establishes and approves budgets for services based upon the individual’s assessed needs and preferred ways of meeting those needs. Manages the fiscal processes related to service authorizations, including use of local and statewide projection tools. Manages processes related to prior authorization as necessary and respond to requests for revisions of authorizations as needed. Establishes and maintains contact with service providers and natural supports as necessary to ensure that services are effectively coordinated by appropriate providers. Promotes community presence and participation. Provides referral and linkage to community resources to help individuals meet their needs and to engage with their communities. Implements an ongoing system of monitoring continuous plan review to ensure that supports are delivered according to each individual plan, that progress is being made toward desired outcomes and that individuals are satisfied with services. Takes action to remediate any immediate concerns regarding health and welfare. Assists individuals in choosing providers as needed. Assists with enrollment onto Medicaid waivers as appropriate. Completes all related paperwork accurately and in a timely manner to ensure maintenance of funding for supports, including development and recommendation of plans for approval to DODD when services included in plans are funded through the DODD Medicaid Waivers. 20% Communications
Furthers the mission of the FCBDD by interacting in a courteous and professional manner with individuals receiving supports, their families, and friends and with co-workers, supervisors, and members of the community. 15% Crisis Intervention & Floating Team Coverage
Works with individuals who need short-term planning and linkage to and other community resources. Assists in coordinating with internal team members, community agencies direct service providers, and system liaisons on crisis situations to stabilize crisis situations by addressing immediate needs and assessing need for on-going planning. Provides ISC case coverage for current or pending vacancies on ISC teams. Provides back-up coverage for highly intensive and wraparound case coordination for people supported. 5% Training
Provides presentations and information concerning the Board, department, and work unit in accordance with Board philosophy and mission. May be asked to provide/participate in activities internal and external to the agency. Participates in and provides in-service training as may be required. 5% Other
Complies with the Safety Rules established for the purpose of fulfilling compliance with Ohio Employment Risk Reduction Standards, Rules, or Orders, through the utilization of protective equipment and decontamination techniques. Performs other duties as directed by the Individual Support Coordination Supervisorand/or Administration. Bachelor’s degree required. Eligible for certification under DODD. Current, valid Ohio Driver's License, points on driving record may not be equal to or exceed 6 and must be acceptable to agency insurance carrier. Ability to work independently. Works cooperatively with others. Maintains attendance to effectively complete functions of the job. Good judgment and decision-making skills. Communicates verbally and in written form in an objective, professional manner. Orally presents information in an objective, professional manner. Ability to interact with families, members of the community, individuals with disabilities and others in a professional manner to promote a positive image of persons with disabilities and the FCBDD. Maintain basic computer skills including word processing, spreadsheets, and databases. Ability to write clearly and concisely. Ability to maintain accurate records and data. Ability to cope with stressful situations. Ability to pass all background checks including Bureau of Criminal Identification and Investigation (BCII). Must be physically capable to lift up to 50 lbs. per lifting policy, move individuals, including children, adolescents and adults in a safe manner, according to in-service training. FLSA
