Floating Individual Support Coordinator

Regular Hours of Work 8:00 a.m. to 4:30 p.m., 37.5 hours per week (may be required to work a flexible schedule).

Department Services and Supports
Responsibilities
Service and Support Coordination (55%): Coordinate person‑centered plans, assess and revise services, manage budgets and fiscal processes, maintain contact with providers, promote community participation, monitor progress and take remedial action when needed.
Communications (20%): Interact professionally with clients, families and the community, representing the FCBDD mission.
Crisis Intervention & Floating Team Coverage (15%): Provide short‑term planning and crisis stabilization, cover ISC teams and intensive coordination needs.
Training (5%): Present information about FCBDD, conduct internal and external training, and participate in in‑service training.
Other (5%): Comply with safety rules, perform duties directed by the Supervisor, and adhere to applicable policies.
Qualifications
Bachelor’s degree required.
Eligible for certification under DODD.
Current, valid Ohio Driver's License with less than 6 points on the record.
Ability to work independently and cooperatively.
Good judgment and decision‑making skills.
Professional verbal, written, and presentation communication skills.
Ability to interact professionally with families, community members and people with disabilities.
Basic computer skills, including word processing, spreadsheets and databases.
Ability to write clearly, maintain accurate records and cope with stressful situations.
Pass background checks, including the Bureau of Criminal Identification and Investigation (BCII).
Physically capable of lifting up to 50 lbs. per policy.
Supplemental Information
FLSA:
Hourly, Non‑Exempt from overtime.
Probationary Period:
One (1) calendar year.
Status:
Classified Civil Service.

Working conditions may include exposure to bloodborne pathogens, communicable disease, and other potential hazards.
