Population Health Manager (RN) – ACO – Morristown
Base pay: $48.00/hr – $84.50/hr
Job Description
Responsible for identifying population health needs and facilitating best practice service delivery to achieve healthy lifestyles and better health outcomes for the population.
Principal Accountabilities
Identifies population health needs, maps services and identifies gaps in healthcare services.
Engages relevant stakeholders (internal and external) to develop and implement service delivery requirements.
Establishes and maintains strategic and program alliances with other agencies whose activities impact public health.
Facilitates and/or contributes to models and plans meant to optimize capacity to meet the healthcare needs of the region.
Collates and analyzes population data from various databases including state and national to support local planning.
Utilizes routinely and purpose‑specific collected data to describe and summarize the health status of the population to identify priorities for public health action.
Other duties as assigned.
Travel required to physician practices.
Qualifications
Bachelor’s degree in nursing required.
Current NJ state licensure as a Registered Nurse required.
3 years of similar experience in a hospital setting preferred.
