Position Overview Working in the home and/or a community environment, a Direct Support Professional (DSP) provides one-on-one services to intellectual and/or developmental disabled individuals, supporting their goals as outlined in the Individual Support Plan. Care includes activities of daily living and collaboration with the multidisciplinary team.
Essential Job Functions
Professionally support adults or pediatric individuals with an intellectual and/or developmental disability to promote emotional, physical, and personal well-being.
Implement strategies to help the individual achieve outcomes in the Individual Support Plan.
Complete documentation (EVV, daily service notes) to substantiate services delivered.
Assist in the development and maintenance of the Individual Support Plan and report any changes in the individual's condition to the manager promptly.
Complete all documentation in accordance with state and program regulations, Av… Healthcare policies, and accreditation standards.
Maintain confidentiality and professional boundaries at all times.
Provide guidance, instruction, and role modeling to help individuals become more independent, including life skills such as cooking, housekeeping, budgeting, and personal hygiene.
Actively seek opportunities for individuals to experience life in their homes and communities based on hobbies and interests.
Ensure health and safety of the individual during all services.
Requirements
High school diploma or equivalency.
Active driver’s license, current automobile insurance, current car registration, and current car inspection if transporting.
Willingness to accompany individuals into the community and maintain sensitivity to civil and human rights and dignity.
Must be 18 or older.
Ability to work flexible hours as needed.
Eligibility for State Police Criminal Record Check and Child Abuse History Clearance.
FBI clearance required for individuals who do not meet PA residency requirements.
Must be able to speak, write, read, and understand English.
