Position Overview Work in the home and/or community environment to deliver quality one‑on‑one services to individuals with intellectual and/or developmental disabilities, helping them achieve their outcome statements outlined in the Individual Support Plan. Responsibilities may include activities of daily living and communication with multidisciplinary team members.

Schedule & Compensation Monday, Wednesday, Thursday – 8:00 am to 4:00 pm Compensation: $18 / hour

Essential Job Functions

Professionally support adult or pediatric individuals with intellectual and/or developmental disabilities in various settings to promote their emotional, physical, and personal well‑being.

Implement strategies to help the individual achieve outcomes stated in the Individual Support Plan.

Complete required documentation (EVV, daily service notes) to substantiate services delivered.

Assist the individual and their interdisciplinary team with developing and maintaining the Individual Support Plan.

Immediately report any changes or problems in the individual’s condition to the manager.

Ensure all documentation is completed in accordance with state, program regulations, Aveanna HealthCare policies, and accreditation standards.

Adhere to confidentiality standards and professional boundaries.

Help individuals reach independence through guidance, instruction, and role‑modeling.

Actively seek opportunities for individuals to experience life in homes and communities based on hobbies and interests.

Support individual preferences and honor their interests.

Assist in developing life‑skills such as interaction, cooking, housekeeping, budgeting, and personal hygiene.

Help individuals live an everyday life while ensuring health and safety.

Requirements

Desire to help individuals live life to its fullest.

High school diploma or equivalent.

Active driver’s license, valid automobile insurance, current car registration and inspection if transporting individuals.

Willingness to accompany clients into the community and provide support during interactions while respecting civil and human rights and dignity.

Minimum age 18 or older.

Ability to work flexible hours as needed.

Eligibility for State Police Criminal Record Check and Child Abuse History Clearance.

FBI clearance required for non‑PA residents.

Function in various environments with potential exposure to allergens.
