Who you are: The LCSW-C (Licensed Clinical Social Worker) Case Manager anticipates and coordinates care/services/health education for targeted member populations with chronic disease conditions or high-cost specialty care. They demonstrate knowledge of complex case management processes and are responsible for ensuring that member goals are achieved within a specified time based upon individual needs, reasonable expectations, with well-documented outcomes.
What you'll do:
Maintain budgeted caseload.
Identify and analyze staff/member/system issues that impact the plan of care.
Identify and act upon impending changes in member condition.
Maintain accurate documentation that meets regulatory standards, policies and procedures.
What you'll bring:
Minimum 3 years of clinical experience.
Managed care experience preferred.
Demonstrated clinical experience.
Knowledge of health care and managed care delivery systems.
Insurance benefit structures and related medical/legal issues.
Utilization management and quality improvement processes.
Member satisfaction.
Education
Master's Degree in Social Work.
Licensure
Current LCSW-C licensure in the state of Maryland.
Certification in case management (CCM) or pursuit of CCM or similar certification within 2 years of hire date.
Salary Range: Minimum $31.92/hour - Maximum $52.69/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
