The LCSW-C (Licensed Clinical Social Worker) Case Manager anticipates and coordinates care / services / health education for targeted member populations with chronic disease conditions or high-cost specialty care. They demonstrate knowledge of complex case management processes and are responsible for ensuring that member goals are achieved within a specified time based upon individual needs, reasonable expectations, with well-documented outcomes. Maintains budgeted caseload. Identifies and analyzes staff/member/system issues that impact the plan of care. Identifies and acts upon impending changes in member condition. Must be able to maintain accurate documentation that meets regulatory standards, policies and procedures. What you’ll bring

Education:

Master’s Degree in Social Work Licensure:

Current LCSW-C licensure in the state of Maryland Minimum 3 years of clinical experience. Managed Care experience preferred Demonstrated clinical experience Knowledge of health care and managed care delivery systems Insurance benefit structures and related medical/legal issues Utilization management and quality improvement processes Member satisfaction Salary

Salary Range:

Minimum 31.92/hour - Maximum 52.69/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
