Job Overview The Case Management Coordinator supports dual-eligible (Medicare & Medicaid) member population by coordinating nonclinical care management services. This field-based, nonclinical role focuses on assisting members with navigating healthcare services, scheduling appointments, accessing benefits, and addressing social determinants of health. The role requires strong critical thinking, communication, and the ability to work independently while collaborating virtually with care teams.
Key Job Duties
Coordinate nonclinical case management activities for members
Assist members with appointment scheduling and care coordination
Identify, educate, and connect members to available healthcare benefits and community resources
Support members with social determinants of health needs
Collaborate with internal care teams using care management tools
Apply critical thinking and judgment to support the overall case management process
Work independently while engaging virtually with cross-functional teams
Required Experience & Qualifications Education
Bachelor’s degree
in behavioral health or human services (Psychology, Social Work, Nursing, Counseling, Marriage & Family Therapy, etc.)
Experience
2+ years
of experience in:
Behavioral health
Or a related field aligned with program focus
Managed Care experience
preferred
