About the Position
WORK LOCATION: COMMUNITY SUPPORT PROGRAM – Wiggins PAY: $33,600.00 annually SCHEDULE/SHIFTS: Monday through Friday, 8:00 a.m.-4:30 p.m., and with variations as required to meet program needs. Responsibilities
Serves as IDD Targeted Case Manager (TCM) for people enrolled in the IDD Community Support Program (1915i). Manages and maintains caseload of people receiving IDD Community Support Program services. Develops and maintains a person-centered Plan of Services and Supports (PSS) for each person on caseload. Revises/updates each person’s PSS at least annually or when changes in the person’s circumstances occur or when requests are made by the person/legal representative. Coordinates and monitors identified supports and services through regular monthly contact, face-to-face quarterly reviews, and annual assessment and re-certification meetings. Ensures services are adequate, appropriate, and meet the person’s needs. Applies pertinent state and federal regulations as well as agency and program policy and procedures. Travel within the six southern counties is required. Must maintain eligibility to drive a state vehicle. Preferred Qualifications
Typically requires a Bachelor’s Degree and 2-4 years of experience. Agency preferred: Requires a Bachelor’s Degree in psychology, mental health, intellectual/developmental disabilities, or related field from an accredited four‑year college or university and at least two (2) years of related experience. Must have a valid MS driver’s license, or license from a contiguous state, and have personal vehicle insurance to drive a State vehicle. Must be eligible to drive a State of Mississippi vehicle.
