We are seeking an experienced Nurse Practitioner or Physician Assistant to provide locum coverage in a home health setting near Fresno County, CA, however, there are multiple locations available.
This is an excellent opportunity for an APP looking for flexible coverage starting ASAP and is ongoing.
Job Details:
Specialty: Primary Care / Home Health
Setting: In-home health evaluations for Medicare Advantage, Medicaid, and select commercial plan members
Schedule:
Flexible schedule: Monday Sunday, 7:00 AM 7:00 PM
No nights or weekends required
Contract Length: Ongoing short-term or long-term options available
Start Date: January 5, 2026 (credentialing required)
Average Visits: 6 9 per day
Charting Software: Client EMR
Work Details:
Conduct pre-scheduled in-home health evaluations
Review medication and medical history
Perform physical evaluations
No prescribing, ordering labs, or altering treatment plans
Requirements:
Active California NP or PA license
Board Certification: Primary Care
Education: Master s degree required
Comfortable performing in-home evaluations independently
No background or malpractice issues that could delay credentialing
Credentialing:
Estimated timeframe: 7 days to 2 3 weeks
Completion of facility-specific forms and verifications required
