Overview IU Health is seeking expert RN’s to collaborate with the brightest minds in medicine. This role blends hands‑on clinical practice with relational community work, ensuring safe, high‑quality care in home, community, and outreach settings. The RN will provide day‑to‑day clinical knowledge, coaching, and health screenings to vulnerable populations, oversee Community Health Workers and other frontline staff, and support access to health, chronic disease management, social determinants of health interventions, and care navigation.
Responsibilities
Provide clinical knowledge, coaching, and health screenings to vulnerable populations.
Supply oversight and operational coordination for Community Health Workers (CHWs) and other frontline staff.
Ensure safe, high‑quality care in home, community, and outreach settings.
Assist individuals, families, groups, and communities in accessing resources such as health insurance, food, housing, quality care, and health information.
Provide informal counseling, support, and follow‑up.
Conduct health screenings (blood pressure, diabetes, lipids) and social determinants of health assessments with care navigation.
Administer immunizations at community events.
Travel to community settings—including homes, schools, and community centers—and perform office‑based duties.
Maintain a valid driver’s licence and reliable transportation to conduct community visits.
Flex up to 25% of time to cover community events during nontraditional work hours (evenings and weekends).
Qualifications
Associate of Nursing (ASN) required; Bachelor of Nursing (BSN) preferred.
Active Registered Nurse (RN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) RN license.
Graduated from a nationally accredited nursing program.
Basic Life Support certification through the AHA (other advanced life support certifications may be required per unit/department specialty).
