Location: Bucks County, PA
Employment Type: Full‑time
Shift: Flexible with weekend rotation once per month and 2 holidays annually
Compensation: Competitive salary + $10,000 signing bonus (eligible for new hires)

Responsibilities
Provide primary nursing care to home‑based patients as ordered by the physician.
Conduct initial assessment and develop a care plan in collaboration with the Population Health Case Manager.
Supervise Home Healthcare Aides and LPNs as needed.
Provide therapeutic interventions and overall care management to achieve top decile client outcomes and patient satisfaction.
Qualifications
Graduate of an approved nursing education program.
Registered Nurse license in the state of PA.
One (1) year professional care nurse experience with home care preferred.
Valid driver’s license and reliable transportation.
