Transition Coordinator – Residential Community Bridges, Inc. (CBI) is an integrated behavioral health care organization offering a full continuum of care, including a variety of programs throughout Arizona, Oklahoma, and the District of Columbia. CBI provides residential, outpatient, inpatient, patient‑centered medical homes, medication‑assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness.

Job Summary Transition Coordinators are assigned at each level of care within CBI’s systems of care network to ensure that patients have an individualized discharge plan. All follow‑up appointments for ongoing care and services will be scheduled within 7 days of discharge. Transition Coordinators coordinate with Utilization Management on all authorizations and continued authorizations, adhere to all policies and procedures, complete an initial assessment and treatment plan, and engage the individuals and their support systems. They provide therapeutic services including counseling, group work, discharge planning, and authorization coordination.

Skills/Requirements

High school diploma or GED required. Associate’s degree in a field related to Behavioral Health preferred.

Minimum of 2 years of full‑time health care related work experience, with adequate exposure to both behavioral and medical situations.

Experience with administrative or operational processes in a health care setting preferred.

Experience working with specialty populations (e.g., SUD, SMI, children, adolescents) preferred.

Valid identification required.

Behavioral Health Technician (BHT) in accordance with A.A.C. R9‑10‑101.33 and CBI Clinical Policies and Procedures required.
