Job Overview The Referral Coordinator is a highly visible customer service and patient‑focused role. The incumbent works directly with our patient population and their families, insurance representatives and outside vendors, physicians, clinicians, and other medical personnel to ensure the referral process runs smoothly and that the highest level of quality healthcare is delivered to our members.
Responsibilities
Coordinate and process patient referrals to completion with precision, detail, and accuracy.
Prioritize HPP patients in Primary Care Physicians panel, stats, expedite and order over 5 days.
Ensure orders are approved when needed.
Schedule patients (Preferred Providers List of Specialists) and notify them of appointment information including date, time, location, etc.
Use Web IVR to generate authorizations via Availity, Careplus, Healthhelp NIA, and any other approved web IVR for authorization processing.
Complete orders with proper documentation on where patients are scheduled and how patients were notified.
Confirm referrals sent to specialist office and receipt.
Prepare and actively participate during physician/clinician daily huddles utilizing RITS Huddle Portal and huddle guide; communicate physicians/clinicians needs or outstanding items regarding patients.
Enter all Inpatient and Outpatient elective procedures in HITS tool.
Ensure patients’ external missed appointments are rescheduled and communicated to the physician/clinician.
Participate in Super Huddle and provide updates on high‑priority patients referrals.
Address referral‑based phone calls for Primary Care Physicians panel.
Complete and address phone messages within 24 hours of call.
Check out patients based on their assigned physician/clinician; if coordinator unavailable, colleague assists (process does not apply to Care Specialist).
Retrieve consultation notes from consult tracking tool.
Follow up on all Home Health and DME orders to ensure patient receives services ordered.
Provide extraordinary customer service to all internal and external customers (including patients and other ChenMed medical team members) at all times, utilizing patient messaging tools.
Perform other related duties as assigned.
Knowledge, Skills & Abilities
Knowledge of medical terminology, CPT, HCPCS, and ICD coding desired.
Understanding of the company’s patient population, including complexities of Medicare programs.
Detail‑oriented with ability to multi‑task.
Excellent phone etiquette and proficiency navigating computer software systems, including technology use.
Team‑oriented and ability to work well with patients, colleagues, physicians, and other personnel in a professional and courteous manner.
Exceptional organizational skills with ability to effectively prioritize and timely complete tasks.
Proficient in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook, database, presentation software).
Spoken and written fluency in English; bilingual preferred.
Ability and willingness to travel locally within the market up to 10% of the time.
Education & Experience Criteria
High School diploma or equivalent required.
Minimum 1 year of referral experience in a healthcare setting required.
Experience with web‑based insurance sites and obtaining referrals/authorizations for multiple payors preferred.
Experience with Web IVRs and obtaining referrals/authorizations for multiple payers strongly preferred.
Healthcare experience within the Medicare Advantage population preferred.
Medical Assistant certification preferred.
CPR for Healthcare Providers preferred.
Pay Range $17.00 - $24.26 hourly. The posted pay range represents the base hourly rate or base annual full‑time salary for this position. Final compensation will depend on a variety of factors including but not limited to experience, education, geographic location, and other relevant factors. This position may also be eligible for bonuses or commissions.
