About the Role We’re looking for a dedicated
Registered Nurse-Physician Practice
to join our team and help us continue delivering the level of care our patients and families deserve. In this role, you’ll organize and administer individualized, professional patient care within a primary care practice. You will provide nursing care as directed by the provider and in accordance with hospital policies, state Nurse Practice Act, and clinical leadership standards.
What You’ll Do
Patient Assessment: Conduct assessments during patient visits, review medications, perform medication reconciliation, obtain vital signs, height, and weight, and document findings in the patient’s medical record. Integrate assessment results into care planning, goals, and outcomes.
Patient Care: Collaborate with the provider to formulate and implement a plan of care based on assessment and diagnosis. Refer patients to case manager or social worker for additional services and resources. Ensure consistent standards of care and meet joint commission standards for patient safety. Triage calls from patients, family members, and community health providers.
Communication: Apply patient‑specific communication skills in culturally sensitive interactions. Accommodate language barriers, sensory, and cognitive deficits as needed. Communicate information to the provider in a timely, concise manner. Manage conflict among team members effectively.
Patient Education: Provide information to patients and families, assess barriers to learning, and consult with the care team to enhance education. Participate in developing patient education materials and provide teaching resources.
Clinical Leadership: Delegate and assign care consistent with scope of practice. Participate in problem solving, provide feedback, coordinate care across the team, conduct research, and use critical thinking to support clinical decision‑making. Ensure staff practice within scope.
Documentation: Ensure all documentation is accurate and complies with institutional standards.
What You Bring
Graduate of an accredited school of nursing (BSN preferred).
Licensed to practice in New York State.
Ability to organize priorities, utilize critical thinking, and multitask while remaining professional, focused, and positive.
Excellent customer service skills with integrity, friendliness, and compassion.
Effective interpersonal communication and telephone skills.
Ability to establish rapport with patients, co‑workers, and medical staff.
Strong written and oral communication skills.
Ability to give, take, and follow direction in a positive manner.
Flexibility to take initiative and embrace new opportunities for growth.
Proficiency with computers for email and electronic medical records.
