Your Job Summary Responsible for fostering a climate, policies and routines that enable residents to maximize their individuality, independence, and dignity. Provide the highest practical level of physical, mental, and psychosocial well‑being and quality of life. Identify psychosocial, mental, and emotional needs and facilitate access to services. Services are provided in accordance with the National Association of Social Workers (NASW) Code of Ethics, and in adherence to federal, state, and local regulations and governing agencies.
Principal Responsibilities
Complete social history and psychosocial assessment for each resident, including relevant parts of the Minimum Data Set (MDS), and use the MDS to guide the care plan.
Maintain accurate and timely documentation complying with federal, state, and local regulations, corporate policy, and center practices (Advanced Directives, MDS, Social Service History and Assessment, RAPs, Care Plans, Social Service Progress Notes, Behavior Tracking, cognitive and psychosocial changes, PASARR, Discharge Planning, etc.).
Support each resident’s right to self‑determination; document refusals and inform residents of alternatives.
Participate in developing written interdisciplinary care plans for residents identifying psychosocial needs, goals, and interventions.
Provide therapeutic interventions to help residents cope with transition and adjustment to a long‑term care facility, addressing social, emotional, and psychological needs.
Educate residents, families, and significant others about placement and facility issues, and refer to appropriate social service agencies when services are not provided by the facility.
Coordinate resident discharge planning and make referrals for home‑care services prior to return to the community.
Assist the Social Service Director/Consultant in planning, developing, organizing, implementing, evaluating, and directing social service programs.
Work with the Director of Nursing and Administration to prepare for inspection surveys, engage with inspectors, and implement immediate corrections of any findings.

Qualifications
Certified, licensed, or registered in the state of practice.
Bachelor’s Degree in Social Work or a related field (Sociology, Special Education, Rehabilitation Counseling, Psychology) from an accredited school of social work.
One year of supervised social work experience in a healthcare setting, directly with geriatric individuals.
Minimum two years of nursing experience in a Skilled Nursing Facility (preferred).
